FAQs

Reoboth House provides a comprehensive service for adults with learning disabilities, mental health, autism. Following a full assessment and preparation of a care plan, care packages are tailored around the clients needs. The aim is to provide an environment as near as possible to independent living, whilst benefiting from the availability of expert support and care as required, this helps to develop personal, social and domestic skills.

 

Those p usually live within a small group of well appointed units and will be supported 24 hours a day by staff available within the same building.

This Supported Living arrangement promotes and achieves a feeling of privacy, dignity and independence for the Service User. Under the Registered Manager, each person’s accommodation is fully furnished, benefits from all modern facilities and is well located to enable easy access to all amenities.

This service provides accommodation and support services for young people aged 18-30, to reach their full potential including those leaving care, unaccompanied asylum seeking children and teenage parents. Following a comprehensive needs risk assessment, and the preparation of a support plan, the focus of the service is the progression of the young person to “move on” and live independently.

 

Our in-house care team focus on matters such as life skills, identifying and sourcing education/training, accessing health and benefits and are available 24 hours a day, including weekends.

Monitored accommodation and support services are provided for individuals aged 19 years old onwards, with a variety of needs. Service is provided on basic three levels, but are flexible to meet the service users needs:

Low Key Support : This accommodation is regularly monitored by a floating team. The monitoring team will undertake planned, routine and reactive visits each week, including out of hours and weekends. Each Service User receives up to four hours of 1:1 key work support each week.

Medium Key Support : This level of service provides a greater and more structured staffing presence within the accommodation, including in the evening and weekends. Each Service User usually receives a minimum of seven hours of 1:1 key work support each week.

We provided unlimited additional support hours as required, including floating support for off site activities. Support services can be tailored to fit individual needs.

High Key Support: This service is for Service Users assessed as requiring the highest level of need and is usually provided with 24 hour staffed accommodation. Service Users will receive a minimum of fourteen hours of 1:1 key work support on a weekly basis. Reoboth House are able to support service users with a variety of needs.

Our specialist team provides support and personal care for adults with learning difficulties, mental health issues, autism, and some challenging behaviour, following a full assessment and preparation of a care plan. Suitable accommodation can also be sourced for client requirement as support packages are tailored around the clients needs.

 

A full range of activities are offered within and away from their home and all our Service Users are given the opportunity and encouraged to engage in a wide range of social activities/hobbies etc. Care packages are tailored to suit individual needs, as agreed between the placing authority, other stakeholders and ourselves and are regularly monitored, in conjunction with the Individual Programme of Assistance/Care Plan.

Self directed care, where the Service User receives and controls their own care budget, so that they can put together their own care package and make their own budget choices.

For some people, this is a major change to the way their care services will be managed and financed and our specialist team is able to provide assistance, advice and guidance to help identify and source the most effective, relevant and cost efficient support and care required. Our team can manage the whole process and also offer more specific support for a particular service, as required.

The range of accommodation provided includes both shared and self contained properties and is available throughout the South London area, predominantly in Croydon, Mitcham, Streatham and Thornton Heath.

To support our tenants and service users, we provide a 24 hour property management and maintenance service and a wide range of specialist housing related and tenancy sustainment support services.

Independent living is an aim of the majority of Service Users, to varying degrees. Our support and care teams will assess all Service Users and make this a focal point of the support/care plan. Our specialist property services teams are able to source a wide range of accommodation, to suit the needs of most Service Users, with support levels adjusted and gradually withdrawn, as necessary.

Reoboth’s support and care teams are diverse and able to address the needs of most people. Projects can be set up for individuals or groups of people, we have a varied accommodation list properties available, many of which can be altered to suit specific Service Users needs. This can range from changing colours and furnishings to being adapted to facilitate a disabled client.

Reoboth House Supported Living is not only an accommodation based support service but fluctuating support is available and provided to eligible people, as required and agreed. Our support team is independently managed, and works in partnership with the housing team/provider to meet the service user’s needs and positive outcomes. The aim is to progress the service user to “move on” and live independently.
