ABOUT REFERRALS

A Homeatpeace Supported Living referral form must be completed and sent to our Intake Team. Once the information has been assessed and any questions answered, a decision can be made as to whether the placement can be progressed, this will apply especially in cases of Service Users with higher support needs.
